Understanding individual and family needs and preferences is essential to providing aged care and palliative care consistent with a person’s values and beliefs. Providing culturally safe care is founded on respect for peoples’ background and lived experiences.
There is an individual and collective responsibility of all those in the health system, including consumers, to identify and reduce any care provided to patients that is of low benefits.
The ELDAC Home Care toolkit has been developed by the University of Technology Sydney with palliative and aged care experts to assist staff working in aged care services supporting client’s at home to provide information and resources on providing palliative care and advance care planning.
