Aged Care Navigator Trial

The Aged Care Navigator Trials are now concluded.

These trials have informed the new Care Finders Program which will commence in all areas across Australia from early to mid-2023.

The Care Finder Program is specifically for vulnerable older people who need intensive support to access aged care and other local services.  To find a Care Finder, go to www.myagedcare.gov.au/help-care-finder

My Aged Care support is also available face-to-face through some dedicated Services Australia service centres.

Previous Trials

July 2020 to June 2021  In partnerships with local community organisations, COTA Queensland delivered group-based information sessions through a mixed modality approach.  This involved COTA Queensland presenters live streaming aged care information presentations and Q&A in to communities, with local coordinators organising the event and supporting people in the community.  Partners were based in Barcaldine, Charleville, Charters Towers, Childers, Cooktown, Gin Gin, Hervey Bay, Hughenden, Maryborough and Pomona.  …
