caring@home

Is a national palliative care project funded by the Australian Government, has launched an updated palliative care medicine app and community guidelines as part of a suite of free resources supporting people who choose to be cared for, and die, at home.

The free prescriber app (palliMEDS) has been updated to include an opioid calculator. This supports switching a patient from some, or all, of their current opioids to a replacement opioid delivered by the subcutaneous route.

Version two of the Guidelines for the handling of palliative care medicines in community services is now available. The updated guidelines include jurisdictional changes and can be used by community service providers to inform the development of detailed protocols and procedures tailored to the requirements of individual services.
